I'm trying to figure out what games all the music disks are from so I can create wiki entries for them.

2. I'm not sure where this plays, but it is on the PSO I&II OST. I assume it was some online lobby or event theme. PSOW states it's just a remix of the lobby theme.

Only 1-12 are originally in BB. The reset were added here on Ephinea. Additionally, 8-12 are all fan-made from a event on JP BB to commemorate the first year of service, 「PSO BB一周年記念感謝祭イベント」, which included several contests, one being a BGM contest.
